Wii Smile Referral Program

The greatest compliment our practice can receive is when our patients refer their friends to our practice. In order to thank you for this honor, we have initiated our           “Wii Smile” program-

 When you refer a friend to our practice, please fill out the form below. For each friend that begins treatment, your name is entered into two drawings:

1)    Annual Limousine Pizza Party

   You and up to eight of your friends will be whisked away from school in your own limo!! You will be taken to a pizza party and then treated to ice cream as you tour the city. Your limo will then drop you off at a predetermined location when your night is over.

    This drawing will occur April 15 

2) Annual Nintendo Wii Giveaway

   At the end of each year, we will hold a drawing to give away a Nintendo Wii system!!

    This drawing will occur December 15.
